c.       Certificate of No Objection if already granted a transfer of credential

C.      RE-ISSUANCE OF DIPLOMA

a.       Original copy of diploma if damaged

b.       Affidavit of Loss if lost

D.      ISSUANCE OF CERTIFICATIONS:

a.       General Weighted Average

b.       Good Moral Character

c.       For PD 907

d.       Medium of Instruction

e.       Course Description

f.        Honorable Dismissal

g.       Theoretical and Clinical Laboratory Hours for Nursing Graduates

                                                               i.      One (1) piece of documentary stamps (worth Php. 30.00) per copy

                                                             ii.      For Honorable Dismissal requirements for the re-issuance of Transcript of Records

E.      CERTIFICATION, AUTHENTICATION, AND VERIFICATION (CAV)
