Below is a Google Form in which you can submit confidential information about your child(ren). If there is anything that you want me to know pertaining to their learning styles, previous experiences, past obstacles, triumphs etc., you can submit this form. You can submit one or as many as you would like throughout the school year. As a reminder, this is confidential.
