Moving Home Minnesota (MHM) is a Medicaid-funded program that helps people move from hospitals, nursing facilities, or other institutions back into community living.
It is designed for individuals who have lived in an eligible institution for at least 60 consecutive days and want support to find a place to live that is affordable, sustainable, and safe.
Benefits are available for up to 6 months following discharge.
Must be enrolled in Medical Assistance (MA)
Must reside in an eligible hospital or facility for 60 consecutive days
Must transition to a qualified community setting

Post-Discharge Support to stabilize housing and services

Confirm eligibility: The individual must be enrolled in Medical Assistance and have lived in an eligible facility for 60 consecutive days.
Submit a referral: Complete the BLRS referral form or contact our team directly.
County and DHS coordination: MHM services require county/DHS authorization before services begin.
Begin transition planning: Once authorized, BLRS coordinates the relocation plan, service connections, and transition timeline.
