STEP 1: DETERMINE ELIGIBILITY
Contact your county to request a relocation assessment. This determines which BLRS service (RSC-TCM or MHM) is appropriate for your client.
Allow between 1-3 business days for a response.
STEP 2: COMPLETE REQUIRED FORMS
Set aside 10-20 minutes to complete the following forms:
Release of Information (ROI) - filled out by person receiving services
Referral Form - anyone can submit a referral
