Selecting instructional and intervention supports is a collaborative responsibility of a school team. The 'what', 'when', 'where' and 'how' intervention details will need to be captured in a meaningful way for clarity, coordination, fidelity reviews, and data-based decision making.


Areas To Capture:

  • Description of each research-based intervention used.

  • When each intervention was initiated and ended.

  • Intensity details such as how often and for how many minutes.

  • How the intervention was monitored.

  • Decisions made as a result of the intervention[s].
