OFLA
Worked for employer for at least 180 days;
Has worked an average of 25 hours per week during 180 days immediately preceding the leave
FMLA
Worked for employer for at least 12 months;
Has at least 1,250 hours of service for the employer during the 12 month period immediately preceding the leave
Own serious health condition*
To care for a family member with a serious health condition* (Spouse/Domestic Partner, Son/Daughter, Mother/Father, Grandparent/Grandchild)
Parental Leave (FMLA, Paid Leave Oregon)/Pregnancy Disability
Taking custody or Placement of an Adopted or Foster Child
Sick Child Leave* (OFLA only)
Extended bereavement (OFLA only)
*Medical Certification must be provided
Navigate to UKG
Click the menu button in the top left corner
Click on My Time > Leave > Request
Select the Reason for the leave, the Start Date, and Estimated Return Date
Select whether your request is for a Continuous leave or Intermittent leave
Click the Submit button in the top right corner

Download the document below and give to your/ your family member's medical provider to fill out.
Your provider can fax completed document to 541-355-1109 or
You can upload the document to your leave request case in UKG
(My Time > Leave > History)

Navigate to UKG
Click the menu button in the top left corner
Click on My Company > Documents > Medical
For Own Serious Health Condition or Pregnancy Disability, please find WH-380E- Self
To Care for Someone, please find WH-380F- Care of
You can take up to 12 weeks (60 days) (it does not count weekends or non-workdays, based on recommendation by the medical provider)
FMLA/ OFLA leave can be taken continuously or intermittently (in as little as 1-hour increments)
These are not paid leaves, but we will use your accrued leave to keep you in a paid status
You will maintain job protection and benefit protection while on an approved OFLA/ FMLA leave
If the leave is foreseeable, you need to apply for a leave at least 30 days before needing it
