Benefit eligible BLAST employees have the option enroll in any combination of the medical, dental and vision plans below.

Full-time BLAST employees receive $1,638.00 per month to use toward the cost of medical, dental, and vision insurance premiums. BLAST employees who work less than full time receive a pro-rated monthly insurance contribution, equal to the employee's FTE percentage, in 10% increments.

Insurance premiums will be deducted from an employee's paycheck each month, on a pre-tax basis.
Rates for all plans are composite, meaning it is the same price to purchase insurance for an individual as it is for a family. If the plans you have enrolled in exceed the district’s monthly contribution amount, the remaining balance will be a pre-tax deduction from your monthly paycheck.
BLAST employees can receive a monthly stipend of $300 by opting out of medical, dental and vision insurance with proof of other group medical coverage*
To qualify, you must do the following:
1. Log into OEBB and decline medical, dental and vision insurance and save your enrollments.
2. Complete the Insurance Opt-Out Stipend form and provide proof of other group coverage.
The form is available in UKG and can be found by going to your UKG > My Info > My HR > Forms > Other Forms
Under the blue "available" tab select the "Insurance Opt-Out Stipend" and click "+add" and follow the prompts.
Upload your other group medical insurance card to the form. Select the paperclip icon on the upper-right side and select "General Health (HR)" when choosing the document type. Click "upload."
* Coverage through the Oregon Health Plan (OHP), Student Health Insurance or the Individual Market Place are not recognized as group coverage by OEBB, and therefore employees with such coverage are not eligible for the opt-out stipend.
All benefit eligible BLAST employees are automatically enrolled in the following supplemental insurance policies through OEBB with The Standard Insurance Company:
Long Term Disability - provides up to 66.67% income replacement for a maximum of 18 months for an approved claim
Life Insurance - valued at two times the employee's annual salary
Accidental Death & Dismemberment - valued at two times the employee's annual salary
Policy premiums for LTD are employee paid, post tax, through monthly payroll deductions. Policy premiums for life and AD&D are paid by the district.
All benefit eligible employees have the option to enroll in additional Life, Accidental Death & Dismemberment, and Long Term Care through OEBB. All BLS employees can access American Fidelity supplemental policies. Click the options below to learn more.
Life insuarnce is available to purchase for benefit eligible employees, their spouse/domestic partner, and/or children.
