Transition planning includes Transition Assessments. According to the Division on Career Development and Transition of the Council for Exceptional Children, Transition Assessment is the "ongoing process of collecting data on the individual’s needs, preferences, and interests as they relate to the demands of current and future working, educational, living, and personal and social environments." The goal of Transition Assessment is to help the student make informed choices, take charge of the transition process and understand the skills needed for post-school environments and employment. There are two types of assessments: formal and informal.
