Medication

For Elementary & Middle School students.  

Also for High School student prescription medications, or for medications that must be stored in the office.

For BEA High School students ONLY.  

This form allows High School students to self-carry and self-administer acetaminophen (i.e., Tylenol) or ibuprofen (i.e., Advil, Motrin) ONLY.  

All other medication, or medications containing additional ingredients (besides just acetaminophen or ibuprofen) must be stored in the office.
