Immunizations

Please use this handy chart from the Minnesota Department of Health.  It lists all vaccines that are required for entry into school in Minnesota.  

All students must show proof of required vaccines or proof of legal exemption to remain enrolled in school in Minnesota.

You may also use the Minnesota Department of Health's Immunization Form to help track your child's immunizations, or use it as a Conscientious or Medical Exemption form.
