Welcome to Athletics Registration!
In order for a Beaver Dam High School student to be eligible for participation in athletics, they must complete the following (5) items listed below PRIOR to the first practice that they participate in. These are numbered for convenience and don't necessarily have to go in order.
1. Updated Physical
Complete a physical as performed by a MD and filed with the Activities Office (Every other year) - form below
OR
Complete an Alternate Year Physical Form and filed with the Activities Office (Years opposite of the actual physical - doesn't require an MD to sign off) - form below
2. Athletics Fee Paid
Each student pays $100.00 to participate. This is a one time fee per year and covers the entire year/all sports within that sports year
This includes past athletics fees - if you have past, unpaid fees, you aren't eligible
3. Registration Completed in Skyward
4. Completed Code of Conduct Video & Verification
5. Completed Concussion Baseline Testing via Sway
This is testing that will be administered at BDHS at no cost to players or families.

Form
Physicals, performed and signed off by medical doctors, are required of student athletes every two years. The form in the link can be printed and taken to the doctor. Please do not use a different form than this.

Physical Form
Alternate year forms are required of students in 'off' years, or the second year of the physical. Physical forms must still be on file with the school, but the alternate year form, which is filled out by a parent/guardian - NOT A DOCTOR - confirms that nothing has changed in the student's health since the previous year's physical. These can be filled out by the parent/guardian and returned to the school without seeing an MD.
