Student Learning Plans are a roadmap for supporting learning environments where students & families can voice their hopes & dreams, identify how they want to engage throughout the year, establish goals for the year, monitor their progress, and get connected to resources.
There are three sections to student learning plans:
About Me - This is where student share their hopes and dreams and how they learn best.
Student Progress, Goals, & Supports - This is where students set academic or interest goals they will complete by the end of the year and lists supports that will help them meet their goals. It also includes information on the student's academic performance and attendance.
Family Collaboration - Strategies, tools, or ideas around how families can engage and support in the learning goals are included in these sections.

What Community School Staff Need to Know
The About Me section of the Student Learning Plans (SLPs) includes a question for families asking if they need help or support with access to resources (e.g., food, uniforms or technology, according to the supports offered by the school)
If the family answers "Yes," there is a checkbox they can select to "Refer to Community School Coordinator"
These meetings and referrals are currently underway
Holly Gonzales will be sharing the referrals with you from your school's SLP process
Community School Coordinators (CSSC) are expected to follow up on these referrals by:
Reaching out to the families who have been identified, sharing that they were referred to the CSC through the SLP process, and asking if they need any resources
Working with the school's SLP Ambassador to discuss how to share the outcomes of the CSC referrals

This option is available to Community School Coordinators who have Infinite Campus access or if there is someone at the school who can run the referral report for them.

You will receive a folder link only you can access
It will contain a password-protected spreadsheet, which includes the referrals
A separate email with your password will be sent to you
