Blue Thunder Band Program Documents

The following documents are required for all students to participate in the the Blue Thunder Band. Students will not be able to rehearse, travel, or perform with the band until all forms have been completed properly.

All medical and informational forms below must be filled out in their entirety before students may participate in any band events.
