12th Grade Students  

Notice of Needed Immunization 

Ohio State Law requires all 12th grade students to receive a meningococcal conjugate vaccine (MCV4). This is a newer requirement.  Students who do not meet this requirement will be excluded from school in the fall of their 12th grade year until they have received the needed immunization.

The immunization can be obtained through your child’s personal physician, at any walk-in clinic such as the one at Walgreens in Barberton, Akron Health Department, and Axess Pointe Community Health Center located on Robinson Avenue (in the same building as Ritzman’s Pharmacy).

You must provide the school with a copy of the shot record.  Just drop it off at the Main Office (Office hrs. 7:00 am – 3:00 pm) or mail it to Barberton High School, 555 Barber Rd. to update your records. Those students who are exempted from vaccines must bring in a form signed by the physician indicating the exemption.
