2026-2027 Athletic Physicals

May 14, 2026 4:30-6:30 pm at Bandera Middle School Gymnasium

Sports Medicine Associates of San Antonio will be at Bandera Middle School on May 14 to administer athletic physicals for the 2026-2027 school year. As a reminder, BISD requires every athlete grades 7-12 to have a new physical at the beginning of each school year. Consent forms will be sent home with all athletes and they are also available by clicking the links below. 

Consent forms must be turned in to Doc Martinez or the athletes coach by May 7. 

The cost is $20 per athlete and no cash or checks will be accepted. Payment can be made on the Rank One Camp Website by clicking here.

Doors will open at 4:00 and physcials will begin at 4:30. All athletes must be checked in by 5:30 (door will be locked at this time).

PPE Release - Physician's release for providing services.  This form must be collected and is the ticket into the physical.  Without this form, a physical cannot be administered.

School Screening Release Waiver - AugustHeart. The August Heart form is for an ECG screen that all athletes are required to get per Sports Medicine Associates of San Antonio. The AugustHeart EKG screen is specifically looking for issues related to sudden cardiac death in athletics.  This screening does not look at issues related to cardiac issues from COVID infections as those types of issues are not picked up with EKGs but require in-office diagnostic scans. August Heart is for current 8th through 12th grade students.

UIL Medical History Form. Please fill out the medical history form only and signn the bottom of the form. Do not write on the Physcial Exam portion of the screen.
