ESY information is required in the following sections of the IEP for any student who is determined to be eligible for ESY. Please refer to the guidance below for the ESY information that is required in each section. Click here for a printable version of this information.

Answer all four questions to determine whether or not the student is eligible for ESY.
A student may be eligible for ESY if:
The student experienced severe regression and require an unreasonably long time to relearn previously learned skills (in this case, the 1st and 2nd criteria below would be marked "Yes"); OR
Predictive Factors indicate that the student is likely to experience severe regression (in this case, the 3rd criteria below would be marked "Yes" and the Predictive Factors Worksheet MUST be completed); OR
Both of the above criteria are met (in this case, all 3 criteria below would be marked "Yes" and the Predictive Factors Worksheet MUST be completed)
Think of this narrative as the statement of services for ESY. Provide a description of the ESY programming the student requires in the box called “Describe ESY Services necessary in order to receive FAPE”. This information will print out as part of the Statement of Services.
INCLUDE:
Type of ESY service(s) required
Amount of service time required for each service. Include direct and/or indirect, # of minutes/hours, frequency of service (weekly, monthly, etc.), and total # of service minutes/hours for the course of the summer.
Transportation needs (indicate whether or not transportation will be necessary)
The ESY services should be individualized to reflect ONLY the services required to prevent severe regression. In most cases, students require less service time during ESY than what is required during the regular school year, since the purpose of ESY services is maintenance as opposed to the goal of progress during the regular school year.
The district's standard ESY program will run for a total of 4 weeks. Students requiring specially designed instruction from a special education teacher will be scheduled to attend the entire ESY program to receive the specific services outlined in their IEP. This will allow for up to 40 hours of special education and related services over the course of the summer quarter. If the data suggests that a student will require more service time than 40 hours in total over the summer in order to prevent significant regression, please contact your ESS Coordinator ASAP to discuss additional options for ESY programming.
Select 'Yes' or 'No' for each of the student’s IEP goals to reflect whether or not the student requires ESY services to address that goal in order to prevent significant regression in that area. Goals marked 'Yes' must align with regression/recoupment and/or predictive factors data. The option to mark the goal as 'Yes' or 'No' for ESY will only be available if ESY is marked ‘Yes’ in the Extended School Year section of the IEP.
DOCUMENT ESY SERVICES QUARTERLY IN THE SERVICE GRID.
A separate line of service must be created in the service grid for each service the student will receive during ESY. Enter services in the service grid as QUARTERLY time for the summer quarter.
Step 1: Select “ESY” in the first drop down on the left.
Step 2: Select the appropriate ESY service from the second drop down on the right (e.g., ESY Audiology Services, ESY Instruction, ESY Mental Health Services, ESY Occupational Therapy, ESY Orientation and Mobility, ESY Physical Therapy, or ESY Speech/Language Services)
Step 3: Select the appropriate Service Provider Role
Step 4: Indicate whether the services are direct or indirect
Step 5: Indicate whether the services will be provided inside or outside general education (NOTE: for the standard ESY program, no nondisabled peers will be present so all services will be outside general education)
Step 6: Enter the total amount of services for the course of the summer as quarterly service time (do not enter weekly service time):
Start date: 6/1/2024 End date: 7/31/2024
Amount: TOTAL # of minutes for that service for the entire summer
Frequency: Select QUARTERLY
