All compliance forms and videos need to be completed by Orientation Day - May 15th, 2025.
Anatomy Lab Orientation needs to be finished before stepping into the lab itself.

The Student Setup Resources for Exxat/Prism aren't required but they are useful for getting your profile created and properly set up.

Please fill out and send this form back to paprog@augsburg.edu before May 15th, 2025.

Medical Clearance Form

Please have your provider fill out and sign this form. You can't substitute copies of your medical records for this form.

You'll upload this to Prism once your profile is set up.

Learn the policies surrounding the PA program and understand the rules and resources of the program. You may be tested on some of the material at Orientation.

Read the Anatomy Lab Rules prior to starting the program. You will be quizzed on some of the material at Orientation.

Student Profile Setup Resources

All compliance documents for your Didactic Phase are stored on Exxat's Prism. Watch the following videos to help you get set up with your student profile. Once you make it to the Clinical Phase, this is where all of your rotations will be published, any hours will be logged, and evaluations will be placed.
