SEBB MEDICAL PLANS
SEBB offers medical plans from Kaiser Permanente, Premera and Uniform Medical Plan-Regence. You can waive your medical if you currently have medical coverage. Medical plan coverages will be listed as subscriber only, or subscriber with spouse, or subscriber with children, or full family. Each tier of coverage will have different monthly premiums, deductibles and out of pocket maximums.
Be sure to research which plan is the best option for you and your family. Things to consider would be:

If you have certain medical needs, you will want to research which plan have the best coverage for your care.
Review the monthly premium and how it relates to the annual deductible. Usually if a monthly premium is lower then the deductible will be higher. Insurance will not begin paying until after your deductible is paid.

SEBB DENTAL PLANS
You must elect dental coverage as this is a mandatory benefit. The district pays the monthly premium for dental in the monthly funding rate for you and your dependents.
There are three dental plans that SEBB offers:
DeltaCare-Delta Dental Managed Care Plan

Uniform Dental Plan-Delta Dental PPO Plan
Be sure to confirm that your dental provider will take the plan you select.
*DeltaCare and Uniform Dental are both Delta Dental. DeltaCare is the managed care plan and Uniform Dental is the PPO plan.

You must elect vision coverage as this is a mandatory benefit. The district pays the monthly premium for vision in the monthly funding rate for you and your dependents.

METLIFE LIFE INSURANCE
In addition to the Employer Paid MetLife benefits, you can opt into Employee Paid Life Insurance and AD&D Insurance. This would be a separate plan that you sign up directly with MetLife. They bill your supplemental plan directly to you with a monthly bill.
You can sign up for Employee Paid Life Insurance and AD&D Insurance, and also make changes to your coverage or beneficiaries, at any time online. Newly eligible employees less than 31 days will have automatic approval for their employee paid enrollment. All other eligible employees who apply after their initial enrollment may have to complete a certificate of insurability with MetLife.
Note: You must name a beneficiary for your Life and AD&D insurance. You can do this online with MetLife, or you can fill out the beneficiary form and mail it into MetLife.

LONG TERM DISABILITY (LTD) INSURANCE
Employer Paid option of LTD will pay $100-$400 per month, this benefit is free for all SEBB eligible employees.
Employee Paid option of LTD will pay either 60% or 50% of your pay (up to $16,667 monthly gross salary) depending on which plan you select.

Health Savings Account (HSA)
A Health Savings Account (HSA) is a tax-exempt account, which means money you contribute is not taxed. When you enroll in the UMP-High Deductible Plan, you are automatically enrolled in the Health Equity Health Savings Account.

The Flexible Spending Arrangement or Limited Purpose Flexible Spending Arrangement minimum annual contribution for 2025 is $120; the maximum is $3,200.
*Limited Purpose FSA is for subscribers who are on the UMP High Deductible Plan & Health Savings Account only. The Limited Purpose FSA can only be used on Dental and Vision.
During annual open enrollment you sign up for the next year's coverage directly with NAVIA. If you are newly eligible or experience a qualifying life event, you can sign up with a paper form that you turn into the benefits specialist.
NAVIA DCAP
The Dependent Care Assistance Program (DCAP) minimum annual amount is $120; maximum annual amount is $5,000 per household ($2,500 each if you and your spouse file separate tax returns).
