Greetings of peace!
Following the news regarding the Inter-Agency Task Force on Emerging Infectious Diseases (IATF-EID) lifting the restrictions for travel for people 18 to 65 years old in Metro Manila, some of our SHS students can now be allowed entry in the campus to get their belongings that they left inside their lockers since the start of the lockdown.
Here are the guidelines that we should follow in retrieving the belongings.
DATES OF RETRIEVAL
The dates of the retrieval will be from January 19 to May 29, 2021 (Tuesdays to Saturdays ONLY from 8 AM to 5 PM). This will be the last extension we will be having with regard to this matter so please plan this trip accordingly.
BEFORE GOING TO SCHOOL
The Campus Safety and Mobility Office (CSMO) is in charge of approving all requests of persons that need to enter the campus. The office requires the following:
a. Visit this link http://bit.ly/verifyslot to verify the availability of slots per day.
i. NOTE: This spreadsheet is UNEDITABLE. You only need to use this to check if there are still slots left for that particular day
ii. In order to ensure social distancing and avoid overcrowding, there will be a limit of 100 visitors who will be approved for each day only. This will be done on a first-come-first-served basis.
iii. If you will be counted beyond this limit, your application for entry will automatically be disapproved.
iv. For those who will be disapproved, you have to schedule for another day and repeat the entire process.
b. Use the following link https://bit.ly/SHSRequest to request for campus access. You need your obf.ateneo.edu email account to access the site. NOTE: This form is only available from 8:00AM to 4:00PM daily.
i. VERY IMPORTANT: Use the code 8888 in the Employee Number field which is assigned for Visitors in the SHS for this purpose, NOT NA (not applicable).
ii. Accomplish the form at least two days in advance prior to your intended schedule of entry.
iii. You will receive a confirmation email that you have successfully completed the form. Please wait for approval.
You will be required to fill up and submit a Health Declaration Form at the gate.
a. The form can be accessed through this link: bit.ly/HealthSurveyForm.
b. This form can be accomplished online, printed out, and submitted at the gate or be filled out at the gate itself, upon entry.
b. You are encouraged to fill out this form online ON THE DAY OF YOUR APPROVED VISIT (filling it out a day before will invalidate it) and print it out before-hand so that you can hand it in at the gate directly and proceed to the SHS without the need to crowd the gate.
In order to facilitate a quicker retrieval, the school is requiring those who will pick up (older siblings, parents, guardians, or any designated representative) to bring the following:
a. Authorization Letter from the student (in duplicate) with the following minimum contents:
i. Complete Name of Student
ii. Complete Name of Representative who will pick up their belongings
iii. Your Gr 11 Strand, Gr 11 Section, and Locker Number
iv. A general list of the contents of the locker
v. Locker Combination of Padlock (if any)
b. Keys to the locker (if any) or the combination
c. Bag for your things
d. A valid ID of your designated representative.
One authorization letter is required for each student.
If the student himself/herself will be the one to pick up, he/she is still required to come to school with the following:
a. Letter to Retrieve (in duplicate) with the following contents:
i. Complete Name of Student
ii. Your Gr 11 Strand, Gr 11 Section, and Locker Number
iii. A general list of the contents of the locker
iv. Locker Combination of Padlock (if any)
b. Keys to the locker (if any) or the combination
c. Bag for your things
d. Grade 11 school ID
UPON ARRIVING IN SCHOOL
Upon approval of the date of your entry, please expect to do the following at the entrance at Gate 3.
a. Fill out and submit a Health Declaration Form or submit an accomplished and printed form.
b. You will be required to submit to a temperature check.
c. You will also need to come to school equipped with a face mask and face shield and bring your own sanitizer.
Parking areas for your vehicles will be coordinated with the CSMO. You will park your vehicles at Bays 1, 2, or 3 in front of the Junior High School. A guard will be assigned here to assist in the traffic in parking your vehicles.
RETRIEVAL
There will be a Help Desk set up at the lobby.
Present the Authorization Letter/ Letter to Retrieve to the person at the Help Desk.
Follow the instructions on which floor to go and to which classroom. Please use the stairs as the elevator will be unavailable at this time.
Please observe a time limit of 20 minutes for retrieval so we can follow physical distancing and avoid overcrowding.
Upon retrieval, leave a copy of the Authorization Letter/ Letter to Retrieve at the Help Desk. This will serve as the Pull-Out Form.
LEAVING THE CAMPUS
Once done, please leave the campus immediately. Please exit at Gate 3.5 ONLY.

In the event that the NCR reverts back to MECQ or ECQ anytime between the said dates above, we will suspend this process until further notice.
If you no longer need the textbooks you will retrieve from your lockers and would want to donate them, please request your authorized representative to put them in the 'used books collection box' in the ASHS lobby.
