Third Party Application Process

If you are hired by a hiring company who is paying you to do work for another company (the third party/CPT supervising company) you must complete two CPT applications. One for the hiring company and one for the supervising company. The employer address and supervisor data on the CPT applications will be specific to each company. The rest of the application i.e. hours, pay, the educational objective, offer letter etc… will be the same. The CPT Employer Info Sheets will be the same EXCEPT for the required signature. You must get signatures from each party. Please submit both applications at the same time.
