A C.A.R.E. plan is a comprehensive intervention plan based on behavior analytic principles and practices. Students on C.A.R.E. plans have been referred by a student's teacher to the school Child Study Team. The Child Study Team then refers the student to the C.A.R.E. team and the team contacts the student's parent/guardian(s) to ask permission to move forward with the referral process. A C.A.R.E. plan is developed in collaboration with classroom teachers, parents, and students. Data collection occurs on a regular basis to monitor student growth.
Students come with a wide range of needs to succeed academically, socially and emotionally in school. The counseling team works together with the student, parents/guardians, staff and administrators, to put together a supportive plan for the individual student, to be successful in school. These plans are created and put into place based on the student's needs, home and school resources, and ability to monitor student's progress. Eagle Creek staff take pride in thinking outside of the box to serve our students. Individual plans may be referred by the Child Study Team, parent, school staff or administrators.
The 504 Plan is a plan developed by a 504 team consisting of parents, teachers, counselor and administrators to ensure that a child who has a disability identified under the law and is attending an elementary or secondary educational institution receives accommodations that will ensure their academic success and access to the learning environment. A 504 referral is needed for the 504 team to process, evaluate and determine eligibility for a 504 Plan. Referrals can be made by a staff member or by the family.
