A Professional Biographical Form must be submitted; form can be found here. Level II teaching license required; ability to achieve Level III by the Internship portion of the program.

The following are the requirements held by the University of New Mexico 

When you have completed your application and are ready to submit it, you will be required to pay an application fee ($60 for domestic applications, $70 for international applications) via credit card online.
