The SST cycle consists of the following phases:

Phase 1: Pre SST

Phase 2: Initial SST

Phase 3: SST in Progress

Phase 4: SST Follow up

Phase 5: SST Review & Referral for Assessment

Phase 6: Support and Services

Pre SST Meeting:

• Complete all information in Beyond SST prior to the meeting.

• Come Prepared

• Confirm Team members attendance

During the Initial SST Meeting:

• Arrive on time for the meeting

• Bring all necessary information

• Encourage participation from all team members

• Create an action plan with a broad range of interventions

• Assign persons responsible for each action/intervention

Post SST Meeting:

• Enter notes in Beyond SST

• Interventions are progressed monitored and action items are followed through

• Schedule a follow-up meeting after 6-8 weeks of targeted, specific, and appropriate interventions have been progressed monitored.
