Membership Application/Code of Conduct Form
Students from other schools may attend the dance as a guest of a student from Fairmont High School. The same expectations are to be followed. The guest may not be older than one year out of high school. This form must be completed and turned in to the office no later than 3:00pm two days prior to the dance. There are absolutely no exceptions. If you show up at the dance without prior permission, the guest will NOT be allowed inside. Thank you for your cooperation!
