Preschool-4 doses of diphtheria/tetanus/pertussis, 3 doses of polio, 3 doses of Hib, 4 doses of pneumococcal, 1 dose of measles/rubella & 1 dose of varicella
K-4th grade-5 doses of diptheria/tetanus/pertussis, 4 doses of polio, 2 doses of measles/rubella, 2 doses of varicella & 3 doses of Hepatis B
Middle School:
7th graders are required to have a the TDAP and Meningococcal vaccine.
High School:
12th graders are required to have a Meningococcal vaccine booster
All vaccinations must be up-to-date unless an exemption form is provided.
