If your child is diagnosed with Asthma or severe allergy/allergies we require an Action/Emergency Plan be completed by your physician each school year.
This better prepares us when taking care of their medical needs.
Medication MUST have original label from the Pharmacy intact.
Ibuprofen, Tylenol, and/or TUMS will be given as needed as long as permission was received on the school registration form. We do, however, not like to give these more than twice a week. This may reflect a more serious cause for concern and we will notify you.
If your child has a diagnosis and warrants a physicians ok for more frequent as needed medication doses, we will require a medication consent form and you will be asked to supply the medication.
Physicals are valid for 13 months
A valid physical is required before students are allow to participate in extra curricular activities
Parent signature is required on the back side of the physical form
School office must have a copy on file
