There are a lot of forms required for this trip and a lot of information to read through. Please take the time to read everything. There are also a lot of deadlines for when the forms need to be turned in. Failure to meet these deadlines for payment and health information could result in your child not attending camp. This is essential for the camp staff to plan and prepare for our arrival. Thank you for your cooperation.
These two forms can be filled out if you have not already done so. They can be sent in as a hard copy or emailed directly to the AMS school nurse, Jody Omohundro (jody.omohundro@amesburyma.org). Please notice that the Camp Bournedale Health Form must have your doctor's signature for prescriptions as well as any non-prescription (over-the-counter) medication. Please read the forms carefully. All students must have an up-to-date physical and immunizations record with the health form. If you have already sent an updated physical to the school nurse, she will attach a copy for you.
You can also have your child's doctor fax the physical and immunizations directly to Nurse Jody: (978) 955-2562
Directions for sending medications to camp
If a parent is going as a chaperone, they can be responsible for their child's medication while at camp.
The Chaperone request form can be filled out and emailed to the trip co-coordinator, Derek Bent (derek.bent@amesburyma.org). Please notice the due date of September 18th. If we get more volunteers than we need, we may have to deny some requests. Get it in as soon as possible. Also, a CORI form is required if you want to chaperone any school field trip. You may have one on file. You can contact Carol Bartlett (carol.bartlett@amesburyma.org) in the central office to see if it still valid. Chaperones do not have to pay for the trip. Your volunteer services are payment enough. Please notice that you have to include a copy of the front and back of your license to be submitted with the CORI form.
The following forms give you additional information about what students should bring to camp. Please note that students are not allowed to bring any phones or any other electronics on this trip. You will be able to contact the camp directly as well as Derek Bent and Mark Rinaldi in case of emergency.
