For the enrollment application and other documents please click this link and print required documents, this does include the document to request a card to verify Tribal Enrollment (commonly known as a Tribal Card/ Tribal ID/ etc). 

Once an enrolled member reaches 18 years of age, parents or guardians can no longer access enrollment documents without permission from the enrolled member.

Don't have your Social Security Card, Certificate of Indian Blood or Certificate of Birth? Below are links to application to retrieve your lost documents. 

DO NOT SUBMIT THESE APPLICATIONS FOR LOST DOCUMENTS TO THE TRIBE, THEY ARE ONLY FOR THE AGENCY THAT PROVIDE THEM

Social Security Card Renewal Application Submit application to Social Security Administration and not to Tribe.

Certificate of Indian Blood Application Submit application to Bureau of  Indian Affairs Agency and not to Tribe. Form expires by the end of 2026

Birth Certificate Application Submit application to Alaska Vital Records Office not to Tribe.
