Don't have your Social Security Card, Certificate of Indian Blood or Certificate of Birth? Below are links to application to retrieve your lost documents. (DO NOT SUBMIT THESE APPLICATIONS FOR LOST DOCUMENTS TO THE TRIBE, THEY ARE ONLY FOR THE AGENCY THAT PROVIDE THEM)
