All students who are new to NC Public Schools, whether its a Kindergarten student or a transfer student from out of state, there are required health records that must be on file. These records must be on file within 30 days of their first day of school. Please contact the school nurse with any questions or if you need assistance.
State law requires that every child entering public schools in N.C. for the first time receive a health assessment. The assessment must have occurred within 12 months prior to entering school. The medical provider, parent or guardian must provide a completed North Carolina Health Assessment Transmittal Form to the school before the 30th calendar day - after their enrollment date. If these required forms are not received by the 30th day, they will be suspended from school until we receive them.
