Transition Form

When your child transitions to St Machar Academy from their primary school you will be required to complete our transition form to ensure that we hold the most accurate data for you and your child and that all permissions are obtained.

Pupil Data Check

We conduct a yearly data check to ensure that the information we hold on your child's record is accurate.  Information requested includes, emergency contacts and health requirements.  We ask that you please notify the school immediately of any changes to the information we hold on file.

Anti-Weapon and Knife Agreement

A zero tolerance approach to the possession of a weapon or knife in Aberdeen City Council schools has been implemented in order to ensure, as far as possible, that pupils, staff and visitors are kept safe from harm.

ICT Acceptable Use Agreement

We explain to all students how to use the internet and school computers in an acceptable way before they are given access.  The ICT Acceptable Use Agreement form contains rules users are expected to adhere to.  We ask that parents/carers discuss these expectations so both you and your child are aware of the expectations that comes with the responsibility of use.

Wearing of Jewellery and accessories during PE

Wearing of personal items of jewellery and accessories by young people during physical activity can constitute a hazard to both themselves and others.  Watches, rings, chains, bracelets and other adornments must be removed before the physical activity begins.  This should become part of the normal routine with staff checking to ensure compliance.

Photography and Video Permissions

During your child's education, photographs and videos may be captured for various educational reasons.  Consent to use these images is entirely your choice by completion of the Photography and Video permissions form.  Consent applies for two years at which time we will request a refresh.  Pupils aged 16+ are able to give personal consent so receive a different form.

Mobile Device Agreement

St Machar Academy has a Mobile Device Policy to tackle improper use of devices at school.  We very much value your support in this matter - ensuring that mobile phones are used appropriately by our pupils, not as distractions from learning.

V4 Annual Block Consent

'Routine and Expected' visits are an accepted part of the curriculum that take place during the school day.  These visits are to local venues within the local school area or city boundary, involve easily managed activities and happen on a regular basis.  We ask that all parents/carers and pupils 16+ complete an Annual Block Consent to cover these excursions to support class activities.
