Physicals

Physicals must occur on or after April 15th, 2025 to cover the 2025-2026 school year. Families must submit a physical form signed by a medical physician prior to participation by a student-athlete. 

Families should submit their student's physical via email to: thobes@aaps.k12.mi.us or golusinm@aaps.k12.mi.us 

Families may also submit paper copies to the main office on the first floor at Tappan once school begins in August. 

Click this form link to download a copy of the MHSAA Physical Form. We also accept any forms used by a medical office. PLEASE MAKE SURE TO FILL IN BOTH SIDES OF THE FORM COMPLETELY!  NOT HAVING IT FILLED IN COMPLETELY WILL DELAY YOUR CHILD FROM PRACTICING/PLAYING A SPORT
