You must print and complete and sign the following 3 Federal and State forms.. Each of these forms must be signed and dated in order to be valid. If they are not signed and dated, we cannot accept them. Your will be sent a secure link to upload these documents.
Federal W-4 Info
The Federal W-4 form tells AAPS how much to pull from your pay for Federal Taxes. Please keep an eye out for the following boxes:
Box 3 Please indicate which status you are claiming for tax purposes. If you do not complete this box, AAPS is obligated to default to "Single".
Box 5 Is a number, namely the number of exemptions you are claiming on your Federal Taxes. AAPS cannot advise you on what number to use for this field. Please consult with a financial or tax adviser if you have questions about the exemptions you are claiming.
Box 6 Is a dollar amount, namely any extra money you would like pulled from your paychecks to be put toward your Federal Taxes. AAPS cannot advise you on what number to use for this field. Please consult with a financial or tax adviser if you have questions about any additional amounts you should pull for your Federal Taxes.
Box 7 Exempt Status, if you are claiming exempt status, be prepared with documentation for that status in the event of an audit. AAPS is a government entity and does get audited regularly.
State of Michigan W-4 Info
The State of Michigan W-4 form tells AAPS how much to pull from your pay for Michigan State Taxes. Please keep an eye out for the following boxes:
Box 4 Your Drivers License or State ID number is required.
Box 5 You may leave blank for now if you do not know, we can confirm this date at New Hire Orientation.
Box 6 Is a number, namely the number of exemptions you are claiming on your Michigan State Taxes. AAPS cannot advise you on what number to use for this field. Please consult with a financial or tax adviser if you have questions about the exemptions you are claiming.

Federal I-9 Info
This form confirms your citizenship status and eligibility to work in the US. Please keep an eye out for the following:
Social Security Number: we do need your full SSN on this form. It is a federal form, we are required to have it fully filled out.
Grey Box in the middle of the page: Preparer and/or Translator please indicate in this box whether you completed the form yourself or used a preparer or translator.
