Identification Card (ID Card) You will have at least two ID cards: your MeridianHealth ID card and your Illinois Medicaid card. You should always carry your card with you. It has important phone numbers. You will need to show it when you get services.• Bring both ID cards with you when you go to the provider • Do not let anyone else use your cards • You may also need to show a picture ID. This is to make sure the right person is using the card •
