Hello!  My name is Susan Moore and I have the privilege of working with your children here at York Middle School as their school nurse.  School nursing is a specialized practice of professional nursing that promotes the well-being, health and academic success of our students.  To that end, my goal is to assist our students to respond positively to normal development, to promote healthy choices and safety and to intervene with actual and potential health problems.  I collaborate daily with YMS staff and families to coordinate care.

Grades 5-8 are a time of rapid change and growth for children both physically and emotionally. It can be a tumultuous time of life as adolescents strive for greater independence.   They need supervision and guidance as they begin to make choices that affect their health and future.  Communication with me about your child's health is key to supporting him or her in school.  Please contact me with any new information or updates about your child's health  status, medications, special needs and your concerns. I appreciate it when parents make time in their busy schedules to drop into the clinic for a chat. I can also be reached during school hours Monday-Friday at 363-4212 or via email at smoore@yorkschools.org

Of special note :  The state of Maine is requiring all students entering or transferring into 7th grade to receive a Tdap booster prior to their enrollment (pertussis, diphtheria, tetanus ). Please  provide proof of immunization from the provider if you have not done so already.  If you are unsure that the school has this required information please contact me for verification.
