PHYSICAL EXAMS:
All grade 9 students are required to provide written documentation of a physical exam by a medical provider. Physicals must be dated February 1, 2024 or later.
Students will not be allowed to attend school or participate in extracurricular activities or sports until written confirmation of a doctor's physical is provided.
Physicals and health information sheets can be emailed to the school nurses: dshu@westwood.k12.ma.us and lcucchi@westwood.k12.ma.us
UPDATED STUDENT MEDICAL INFORMATION:
Please update the health office with any changes in your child's medical condition
Sports Participation:
Written documentation of a current physical exam is required for participation in interscholastic sports. A physical exam is current for thirteen months from the date it is issued.
Students may not participate in a sport until a current physical is on file in the nurse's office and the student has registered for sports online at FamilyID
