New Students/Transfers:
New students in all grades must have written documentation of a valid physical exam done by a physician within 6 months of entering school.
Written documentation of current immunizations signed by a physician is required before entry to school.
Students from outside of the US or Foreign Exchange students may have additional requirements that will be determined by the Massachusetts Department of Public Health.
Parents must complete and return the Westwood Public Schools health information sheet to the school nurse. Parents need to notify the school nurse in writing of any medical conditions the student has that may affect their well being, including any religious stipulations regarding restrictions in medical care.
No visiting or exchange students may enter school until these requirements are complete. Parents of incoming students should speak to or meet with the school nurse prior to entering school to make sure that all requirements have been met.
