- Verification of at least 5 hours of Volunteer work with a Veteran Service Organization, or towards an event through another organization that directly affects Veterans or the Veteran Community.
- Copy of family members DD214 for proof of relationship and letter from family member verifying relationship (please black out social security information and any other information Veteran does not wish to be seen)
- Essay to explain what your veteran family member has meant to you, goals, and need for scholarship. (Please remember, this goes to a committee for review, please do not use names of family members if mentioning them in the essay)
