I had chicken pox when I was a kid. Do I need the immunization? NO, but you need to get a Varicella titer done, at the Health Center. If your titer is low, you will need to get immunized.

I had the Hepatitis sequence as a kid. What do I do now? Contact your doctor to provide us with the dates of the series.

Where do I get a money order, and how do I fill it out? Will the State accept a personal check? No, they won't. Go to your bank. The top line is the date, the remaining lines are self explanatory. You can also get a money order at Tom Thumb.

I'm not sure how to answer some of the questions on the Trainee Application. Download the form and save it. Type directly into it. The standard answers for UWF have already been provided. Bring up questions to the program director or the clinical site coordinator. The application will be reviewed before it is delivered. Keep a completed copy in your files.
