Required Application Materials
The MDE Application for an Addition to a Minnesota Education License. This can be downloaded at the MN Department of Education Licensing website, under the heading "Forms for processes not yet available online."
The Conduct Review Statement. This form is part of the Application listed above.
A copy of your current Minnesota teaching license.
A check made payable to the “Commissioner of Education.”
Official transcripts, in the sealed envelope, from other colleges or universities, if you have taken coursework towards the added license following admission to the license program. A University of Minnesota transcript is not required.
Official MTLE test score results sheet, if you did not have them reported to the University of Minnesota. We will return your official score report to you.

Licensing Timeline
Complete application packets are reviewed, signed, and mailed to the Minnesota Department of Education within two weeks.
This timeline maybe extended during the busy summer season. The Minnesota Department of Education (MDE) will typically take from 8-12 weeks for processing of your license.
