LEARNING OBJECTIVES - Basic Clinical Training (BCT Trainee)
1. Introduction
It is anticipated that the PGY-1 resident will arrive with an adequate core of basic pediatric knowledge and skills, acquired in undergraduate training, including:
The ability to take a pediatric history and perform a physical examination, including a developmental examination.
The ability to develop and produce differential and provisional diagnoses
Knowledge and understanding of the pathophysiology, epidemiology, assessment, investigation, and treatment of the common pediatric disorders.
Familiarity with unique aspects of pediatric care such as immunization, nutritional and feeding requirements, screening for handicaps, accident prevention, etc.
2. Knowledge
At the completion of this rotation, the resident should have acquired an understanding of:
The appropriate pediatric history and physical exam, including growth and development milestones of children.
Neonatal / pediatric resuscitation
The etiologies, significance and treatment of fever and infection in the child
The pathophysiology, assessment and management of fluid / electrolyte and acid / base disturbances in the pediatric patient
The specific problems and unique features of pediatric trauma
Toxicologic syndromes in the child
The mechanisms of action, indications, contraindications and dosage of drug therapy in pediatrics, including resuscitation drugs
The recognition, assessment, investigation, and management of child abuse / neglect
The reporting requirements of the Child Protection Act
The social agencies available for psychosocial disorders
The principles of pain management in the child
3. Enabling Activities
To achieve the objectives the resident should be expected to demonstrate:
Appropriate knowledge and skills in the initial evaluation of pediatric patients with medical or surgical problems by obtaining an adequate history and physical examination which is pertinent to the presenting complaints and age of the child
Appropriate knowledge and skills in the recognition and treatment of common pediatric illnesses and injuries.
Appropriate knowledge of development milestones expected for children of certain ages
Appropriate knowledge and skills in the psychosocial aspects of pediatric care including the recognition of the family at risk for child abuse and neglect
Appropriate knowledge and skills to evaluate and treat, as well as delivery of ongoing care, to pediatric patients with medical problems, including but not limited to acute fluid and electrolyte imbalances, respiratory insufficiency, infectious processes, poisonings, cardiac disturbances and metabolic problems
Pediatric airway management skills
Knowledge and skills involving intravenous access for pediatric patients
4. Procedural Skills
The resident should understand the indications and contraindications, local and systemic complications, and ideally have an opportunity to perform the following skills:
Bag – valve – mask ventilation
Venous access – peripheral, umbilical, intraosseous
Lumbar puncture
5. Clinical Application:
Critical appraisal of relevant literature and application
Bioethical issues
Medicolegal aspects
Quality assurance / risk management issues
6. Physician Relationships and Communication:
Communicate effectively and compassionately with the patient and family
Understand the importance of a multidisciplinary team and interact effectively with physicians, nurses, and other health professionals
