Health Concerns

If you have not filled out and returned a completed Health Summary please do so ASAP. It is important for us to know of any health concerns so that we can assist in providing a safe environment for your child and communicate with teachers to provide assistance with your child’s learning. A doctor’s order must be provided if your child has restrictions in their diet or activity. Please note that a child’s dislike of a particular food or drink cannot be considered as a restriction. All children will be required to take what is on their meal plate, unless a written doctor’s order is provided.

Absentees

If your child will be absent from school or will be late to school we ask that you please call and let us know. If we do not receive a call from you, the school nurse will be calling to check on the wellbeing of the child.

Emergency Information

Please make sure that you have provided your child’s school with updated phone numbers and emergency contacts in case your child is sick or injured. If changes occur throughout the year please call to update your contact information.
