Transcripts

If you would like to request an email copy of your transcript, please complete this form and email it to info@mhbx.org with a picture of your photo ID.  If you would like to request an official hard copy of your transcript, call us at 718-466-6800 or email us to schedule an appointment for pick-up. If your workplace or school is requesting an official copy of your transcript, they can email us with the release form.
