1. Complete the Student Information & Policies Agreement Form (click here) to order their t-shirt. 

2.  Pay your singer's choir fee using the MySchoolBucks Store. (Links are COMING SOON!)

JV Choirs (Total $45) Activity Fee: $35 (link) T-Shirt Fee: $10 (link)

Payment Information

Complete the payment through MySchoolBucks. If unable to complete the payment online, cash or check will be accepted.

Please ensure that all forms of payment turned into the choir program in a sealed envelope with student's full name

and what the payment is for. 

Check Information:

-Make checks payable to "LMS Choir"

-Write your students name & what the payment is for in the subject line

-Write your DL Number in space available on check per LISD Guidelines

3. Complete the required travel forms through RankOne (click here).

This step is required for all members of the LMS Choir program. Physicals are not needed to participate in choir.
