Medication Forms

MEDICAL FORMS AND INFORMATION ABOUT MEDICATION WILL BE DISTRIBUTED IN MARCH, 2024.  Below is additional information.

***Students who take medication in the morning should take it BEFORE arriving at the school on Wednesday May 8.  This is especially true for any motion sickness medication!  We we will distribute medication the rest of the day Wednesday, and all day Thursday/Friday.

Students are NOT allowed to have medication on their person or in their bags while on the trip.

PRESCRIPTION MEDICATION FORMS:

Students needing and planning on taking prescription medication with them to Washington DC must fill out the appropriate paperwork AND get a doctor's authorization for EACH prescription medication taken on the trip.

NON-PRESCRIPTION MEDICATION FORMS:

Students wishing to take non-prescription medication with them may do so by listing the medication and dosage on the back of the "Extended Travel Permission & Medical Form"

MEDICATION:

The chaperones authorized to dispense the medication will collect the medication before the trip and distribute the medication while on the trip.

ALL MEDICATION must be in the original Rx bottle or for non-prescription medication, the smallest unopened bottle possible. (travel size preferred!)  All bottles must be in a ziploc bag with the student's name printed on the outside of the bag and on all bottles.

Inside the plastic bag with medication, there must be a paper with the each medicine and exact dosage instruction as per parent/guardian instructions.
