Educational Facilitators cannot place orders for a learner until the Written Agreement, Acknowledgement of Responsibilities, iLEAD Exploration Agreements form (including the Community Provider and Family Service Agreement and Learner/Family Guidebook), and High-Risk form (as applicable) are signed by the learner and parent/guardian.
All order requests must be submitted to your Educational Facilitator (EF) in writing via an Order Request form. The only exception to this rule is Amazon. If you are requesting an Amazon order, please send your Amazon Wishlist to your EF. For detailed Amazon ordering instructions, please visit the Special Community Providers page of this handbook.
To submit a Materials or Service Order Request form, please log in to the Hub and navigate to the Ordering section. Your EF will review your order request, and if approved, will submit a purchase order to the billing department for final approval.
Ordering Materials
When requesting materials, submit your request using the Materials Order Request form. You will need to include the provider name, item number, item title/description, the URL for the item, and its cost. Incorrect/unwanted materials requested by the parent and ordered by the EF are non-returnable.
If an item you requested is not available or back-ordered, it will be canceled and you can try to order it again at a later time. If materials for multiple siblings need to be ordered from the same material provider, a separate Materials Order Request form must be completed for each learner. Material orders cannot be combined to save on shipping costs. Please keep in mind that the pricing information listed on a provider's website is subject to change.
Ordering Services
When submitting an order request for a live class or lesson, submit your request using the Service Request form. Confirm the class/course price and dates as you will need this information to submit your order. A maximum of $500 per provider per month for services can be requested. Core curriculum and intervention needs are prioritized. Before allowing your learner to attend lessons/classes with a service provider, you must confirm with the provider that they have received a purchase order from the school. If a learner attends classes without an approved purchase order, the parent will be responsible for any provider charges.
