Go here to get started. Just creating an account is not enough, though. You must also follow the directions below to make sure all of your paperwork is up-to-date and properly documented through DragonFly.
Just once (so if you've done it before at any point, you can ignore this step), each player needs to complete this sportsmanship program: https://nfhslearn.com/courses/37000/sportsmanship
Once the program is complete, players should print a copy of their certificate, take a picture of it, upload it to DragonFly, and then hold onto the hard copy for future reference.
Before the beginning of each year, you must do two things:
1) Complete the required paperwork on the DragonFly app/website.
Make sure you complete all of the paperwork (including the bit related to the field trip permission form).
(Note: if you have a current physical but don't know the date on it, just enter yesterday's date when completing the online paperwork--and Mrs. Clark will make corrections for you from there. Also, the immunizations info is unnecessary, so just skip that part of the process.)
2) Upload a picture of your medical insurance card to DragonFly. (You must do this every year, even if the card is the same one you uploaded last year.)
When a player's physical expires, a new one (on the proper form: Physical Form--Revised 2018) must be completed. Players should take a picture of their physical and upload it to DragonFly and then hold on to the physical copy for future reference.
Note: The physical form must be completed by a doctor (not a physician's assistant or nurse or anything), both the parent and student must sign the physical form, and we must make sure that the doctor dates the physical. The physical form consists of 2 pages. BOTH pages need to go to the Dr.’s office. Page 1 is the page that the parent/athlete completes that has their name, address, date of birth, etc. and all of the questions they are supposed to answer. This needs to be done prior to going to the Dr. office and signed by both the parent/guardian and the athlete along with the date. The Dr. needs this page in order to complete the physical on the back page. It is the page that the Dr. signs and dates at the bottom of the form. Please make sure before you leave the office that the Dr. dated the form. When physicals are turned in without a parent/athlete signature or the date of the physical by the Dr., it can slow up the eligibility process for your student-athlete.
The ONLY time you should complete the electronic physical form on DragonFly is if:
Your athlete is having an in-school physical or
You complete it electronically, print it, and take to the Dr. office with you for the physical.
The Dr. can not complete physical on your student-athlete without Page 1.
