Working Papers

To obtain your working papers, you may stop in the guidance office during any school day and speak with Ms. Jelley about completing these. There are two forms - You may also download the application form here and the physical certification form here.

Part I on the application needs to be filled out by student, with a parent's signature, the rest will be completed in office with Ms. Jelley. Students should also come knowing their social security number for the formal certificate.

Part 2-The Physical Fitness Certificate must be filled out by the child's pediatrician, or by the school nurse (only if the school has a current physical on file for that student).

Students applying for working papers must be in good standing with behavior and attendance, and have a passing gpa.

Note: If you are leaving school for full-time employment, Part 5 should be filled out by the Counseling Office secretary.

Senior Work-Release Program

Seniors at Hoosick Falls Junior/Senior High School are eligible to participate in the Work-Release Program.

To obtain your approval for the Work-Release (Employment), please follow the steps below:

Pick up the Application for Senior Work-Release Program from the Counseling Office or you can print the form.
Fill out your First and Last name, read the requirements and Sign/Date the Student signature section.
Get the form completed by your Parent/Guardian, Employer and the HS Principal.
Bring the completed application back to the Counseling Office.
