Below please find the application forms to either renew an existing Diversification designation, or to apply for a Diversification designation for course that does not currently have one. Use the Diversification Renewal Form to renew an existing Diversification designation for a course. You may also use this form for a course whose alpha or course number has changed but whose fulfillment of the Diversification hallmarks remains the same (e.g., Drama 101 changing to Theatre 101; or Art 106 changing to Art 116, etc.). Renewals are good for five years.
If you are applying for a Diversification designation for a course that does NOT currently have one, use the Diversification Course Request Form. Please note that a proposer seeking a Diversification designation should have secured approval for the course from the proposer's Division, Division Chair, the Leeward CC Faculty Senate, Vice Chancellor for Academic Affairs, and Chancellor PRIOR to application. In short, the course must exist BEFORE applying for a Diversification designation. Petitions for Diversification designations that have not completed the approval cycle prior to application will be rejected, and proposers will be asked to resubmit their applications at a later date. Additionally, experimental courses are NOT eligible for Diversification designations as they are considered temporary rather than permanent courses. First-time Diversification designations are good for five years.

Please note that we do not currently have sample initial applications and attendant syllabuses for each Diversification designation. We will work to add more in the future. However, the available sample applications and syllabuses may provide useful frameworks or ideas when composing your own.
