I usually don't bother with the distinction between delete and delete and close up. That's a fairly refined distinction for type-setting.
Standard proofreading practice correlates inline marks (such as triple underlining for caps) with a mark in the margin. I typically circle the element to be changed and put the mark in the margin.
I also use squiggle underlining to signal a word or phrase to which one of my custom abbreviations (below) applies.
