Physicals and Immunizations

Physicals:

It is important to obtain medical examinations at your child's health care provider's office. These physical exams should be completed at least once during each developmental stage.

Early childhood- Pre K-3rd

Pre-adolescence- Grade 4-6

Adolescence- Grade 7-12

Health Insurance:

Families that do not have health insurance should contact NJ Family Care to apply for coverage. Visit www.familycare.org or www.healthcare.gov for more information. NJ law requires all children to have health insurance.

Immunizations:

Pre K and Kindergarten immunization records must be provided to the school nurse prior to the start of school. Please be sure that all required immunizations are given prior to submitting. Students will not be admitted to school unless all required immunizations are up to date.

All students entering Grade 6 and 11 years old, must present proof of Meningococcal and Tdap vaccines.

Students who received a Td booster dose less than five years prior to entry shall not be required to receive a Tdap dose until five years has elapsed from the last Td dose.

Please provide an updated copy of your child's immunization record each time new doses are given.
